MCGSA is maintained and operated by volunteers. In order to continue providing a vibrant program we count on volunteers to contribute their time, energy and talents. There are opportunities to serve the MCGSA organization and in your child's league(s). Please choose an area where you can serve below.

By completing and submitting this form, I, being the parent/guardian of the child named in this form, do hereby consent to her participation in any and all MCGSA activities. I assume all risks and hazards incidental to such participation including transportation to and from the activities; and to the full extent permitted by law, agree to defend, indemnify, release, and save harmless, Murphey Candler Girls Softball Association, DeKalb County, any and all coaches or instructors, and each of their respective officers, directors, members, volunteers, independent contractors, agents, representatives, employees, participants, successors and assigns, from and against any suit, claim, cost, expense, damage, loss, or liability of any kind or nature whatsoever (including attorneys' fees), caused by or arising out of my child's participation in any MCGSA activities, whether or not caused in part by the active or passive negligence or other fault of a party indemnified hereunder, except to the extent and in the amount covered by supplemental medical payment insurance. I also convey the authority to said association to obtain medical attention for her in my absence should the need arise. I understand that I am liable for the expense of such medical assistance; and that, the ASSOCIATION provides only supplemental medical payment insurance.
